Focused Care is exclusive to Palo Alto Commons and features a secure and supportive environment for residents who require more focused care services and extra assistance with their daily care needs. Focused Care is a unique and flexible option for residents with a high level of care needs but varying social and dining needs.
In Focused Care, we provide loving care for residents who need extra assistance with their Activities of Daily Living (ADLs). The higher staff-to-resident ratio allows for more extensive care and assistance, while residents engage in social and dining programs in the Assisted Living, the Meadow Wing, or the Focused Care program (depending on their level of functioning). Ultimately, this unique program ensures the same high quality of life that our more independent residents enjoy.
All assistance with Activities of Daily Living and many specialized licensed nurse services are included in the monthly Focused Care fee. We develop a Personalized Care Plan, tailored toward each resident’s individual needs.
Our Focused Care program is staffed 24 hours each day with a staff-to-resident ratio that far exceeds federal and state regulatory standards. The 24-hour licensed nursing staff supervises the Focused Care program and provides many licensed nurse services. In Focused Care, we have the capacity to use mechanical devices, such as the stand lift and the Hoyer lift, for assistance with transfers. Also, staff can provide full assistance with dining needs in the Focused Care dining room. A shower room enables our staff to provide shower assistance to residents in a safe and comforting environment.
Focused Care features Friendship Living: shared living accommodations where residents share the simple joys of life’s daily routine. Private apartment residences are available as well. Display of personal memorabilia and photos is encouraged, as is decorating private space with favorite furnishings and treasures. An unobtrusive security system also ensures a restraint-free, safe environment for residents who need extra supervision.
